In the event that my minor child requires medical treatment while participating in activities sponsored by Connecting Point Church of the Nazarene, I hereby authorize the staff or designated representatives of the church to obtain and consent to medical treatment for my child as deemed necessary, including but not limited to first aid and transportation to a medical facility.
I understand that every reasonable effort will be made to contact me or my designated emergency contact if an emergency event were to occur. However, in the event of an emergency where I cannot be reached, I grant permission for medical personnel to provide treatment as deemed necessary for the health and safety of my child.
My child has permission to participate in activities associated with Vacation Bible School at Connecting Point Church of the Nazarene, scheduled for June 8–12, 2026.
Assumption of Risk
I understand that participation in church activities may involve certain inherent risks, including but not limited to minor injuries that can occur during games, activities, or other group events. I voluntarily assume all risks related to my child’s participation in these activities.
Release of Liability
I hereby release, waive, discharge, and hold harmless Connecting Point Church of the Nazarene, its pastors, staff, volunteers, leaders, and representatives from any and all liability, claims, demands, damages, or causes of action arising out of or related to any injury, illness, accident, or loss that may occur during my child’s participation in church activities, except in cases of gross negligence or willful misconduct.
